Health Card

Health cards are cards that you MUST have to access any health services including emergency/urgent care, clinics, eye care, dental, and immunization. 

 

The Health Card contains all of your personal information such as your name, birth date, your 6-digit family or person's registration number, plus your 9-digit Personal Health Identification Number (PHIN). The PHIN is what you usually need to use to identify your personal health info.

 

To apply for a health card you must:

  • Be a Canadian citizen, resident, or a work permit holder (and their spouse/children)
  • Have a permanent residence (house/apartment) in Manitoba,
  • Stay (physically) in Manitoba for at least six months in a calendar year
