Vaccination Cards/App

For those who haven't gotten their vaccination cards yet, please visit this website to get one:

https://immunizationcard.manitoba.ca/#/digitalcards 

They will ask you for your email to create an account. Then you will enter your information (PHIN, name, etc.) and your card will be sent to you within a few weeks.

If you don't have a health card or a PHIN number then call the number below and they will give you the info you need to get your card.
